Pediatrics Day and Night requires that every patient within our group receive all vaccinations as recommended by the CDC.

We will be unable to see anyone who does not adhere to their guidelines, for it will put our other patients at risk.

New Patients are required to provide our office with their vaccination records at the time of their first scheduled appointment, failure to provide them will require our office to reschedule your appointment.

Using your secure password and user ID, you can log into our practice at MyKidsChart.com/pbn , 24/7 from your home or office. All you need is a mobile phone, tablet or computer with internet access.

Information You Can Access Through My Kid’s Chart When you access our patient portal, you can review a complete health information summary for each child in your care, including:

• Most recent physical date

• Upcoming appointments

• Historical visits

• A summary of labs and medical tests

• A problem list, allergy list and medications list

• A complete immunization record

You can download or print your child’s immunization record directly from Pediatrics By Night’s portal at MyKidsChart.com/pbn , and you can review a detailed visit summary for any appointment.
